Bilingual Patient Coordinator (English & Spanish)

About the role

We are seeking a highly organized, customer‑focused Bilingual Patient Coordinator to act as the primary point of contact for patients receiving home health services. Working fully remote as an independent contractor, you will support patients, families, Home Health Aides, and clinicians during U.S. Eastern business hours.

Key responsibilities

• Coordinate work assignments based on Home Care Worker training and experience.
• Communicate effectively with patients, families, HHAs, clinicians, contracts, and internal departments.
• Document all communications accurately in HHA Exchange.
• Monitor and resolve open cases, ensuring timely resolution.
• Provide updates on service changes, discharges, and other relevant information.
• Prepare daily, weekly, and monthly reports and manage overtime/travel‑time requests.
• Use Smart Map and other tools to locate aides, give directions, and check availability.
• Complete incident reports, manage mutual and linked cases, and respond promptly to emails, voicemails, and faxes.
• Follow agency guidelines, policies, and procedures, and escalate issues as needed.

Required profile

• Fluent in English and Spanish.
• Excellent written and verbal communication skills.
• Strong customer‑service and active‑listening abilities.
• Highly organized with strong time‑management and multitasking capabilities.
• Ability to work under pressure in a fast‑paced environment.
• Positive, team‑oriented, flexible and adaptable attitude.

Required skills

• Experience with HHA Exchange.
• Proficiency using Smart Map and other healthcare software tools.
