Bilingual Clinical Care Coordinator

About the role

We are looking for a bilingual Clinical Care Coordinator to remotely support a U.S.-based healthcare practice. The role bridges patients, providers, and insurers to ensure timely, coordinated care.

Key responsibilities

• Communicate with patients about appointments, treatment plans, follow‑ups, referrals and other care needs.
• Coordinate care across providers, specialists, pharmacies and facilities, and maintain accurate records in the EHR/EMR.
• Assist with insurance verification, eligibility checks, prior authorizations and referral scheduling.
• Conduct patient follow‑up and care‑management calls, including Chronic Care Management when required.
• Document patient interactions, clinical information and provider instructions while ensuring HIPAA compliance.

Required profile

• Clinical background (Nurse, Doctor, PA, Medical Assistant or similar).
• Strong understanding of medical terminology, clinical workflows and HIPAA requirements.
• Excellent English and Spanish communication skills.
• Ability to work independently in a remote environment while managing multiple patient priorities.

Required skills

• Experience with EHR/EMR systems.
• Insurance verification, prior authorizations and referral coordination.
• Chronic Care Management (CCM) processes.
• Use of telehealth platforms such as Office Ally.
• Patient communication tools like RingCentral.
